FEES, PAYMENTS, AND INSURANCE BILLING:
I am a behavioral health care provider with several major insurance companies.
It is your responsibility to call your insurance company PRIOR to your initial evaluation to find out if my services are covered under your health insurance plan. Import things to ask your insurance company is 'eligibility', 'deductibles', and if you need to 'precertify' my work with you. Some insurance companies will not pay pay all if the care is not 'precertified'. It is your responsibility to notify me, if I need to assist you with any precertifications.
Co-pays, deductibles and/or full payment for sessions are due at the time of your session. I do not carry forward balances for co-pays and deductibles.
Full disclosure statement for your review regarding my policies on cancellations and missed appointments will be sent to you prior to your first appointment.
